Adult Services are provided to approximately 44 individuals ranging in age from 20 to 62 who have been diagnosed with intellectual disabilities and other developmental disabilities. All individuals have been referred by the Division of Developmental Disabilities and reside in small group home settings, duplexes or their apartments, which are located statewide.
Each individual takes the central role in shaping the quality of his or her own life by active participation in all aspects of planning and follow through of services. The individual in conjunction with their support team monitors services.
Staff supports may range from ten hours a week to twenty-four hours per day. The role of staff is to support individuals in achieving desired outcomes. In addition, professional services are provided on an as needed basis. They may include nursing, counseling, psychology, occupational, physical and speech therapy.
PROGRAM SETTING:
Seven residential facilities provide 24 hour supports for the majority of our adult clients. 99 Willow Street in Providence is a Supported Living Program for adults with disabilities through the Federal HUD 811 program. There are currently 20 people living in 12 apartments. Each person is capable of living without 24-hour supports but receives anywhere from 6 to 24 hours of support for daily living or vocational needs.
Service emphasis includes community participation, increased social interactions, access to competitive employment or alternative day options, development of home living skills, improved medical and emotional status, and living environment that provides as much independence as possible.
REFERRALS, DECISIONS, AND INTAKE:

Referrals: Referrals are accepted from the Division of Developmental Disabilities. The required intake/referral information includes eligibility status and a personal capacities inventory provided by the Division as well as available information in the following areas; social case history, psychological evaluations and/or information, work history, medical evaluations. Occupational, physical, and speech therapy, psychiatric evaluations, and any progress reports or IP’s from the referring agency or hospital. The multidisciplinary team prior to intake reviews the referral information.

Interviews: Once the referral material has been reviewed and it is determined acceptable, dates and times are scheduled to meet with the individual, family members or guardians, DDD social case worker and referring team members. In all cases, the nurse and psychologist are actively involved in the interview process.

Pre-placement: Once suitability of referral is determined, visits including overnights are scheduled. Some individuals may require more visits than others before a final determination is made. Upon completion of visits a decision, with input from the individual, is made to determine an intake date. In general an individual will visit 3 to 5 times prior to intake. The Program Manager is responsible for the admission process.

Intake: The formal intake includes meeting with individual, family or guardian as appropriate and members of the multidisciplinary team to develop a transitional individual support plan, support agreement and budget to be submitted to the Division of Developmental Disabilities. Once the budget is approved a date to move into the facility is set. All agency and regulatory documents are signed during this time

STAFF SERVICES
Staffing: All adult staffing in based on individual need. Persons may be serviced from 10 hours per week to 24 hours per day based on need and funding provisions. All individuals serviced in our group home settings are shift staffed on a 24-hour basis. Administrative support, supervision or backup is available 24 hours per day by managers, administrative or clinical personnel as needed. The staff/participant ration ranges from 2:4 to 1:1 based on individual needs. All homes have at least one awake individual on during the overnight hours.
